April 9, 1943
the entire crew was aboard
B-25D "Little Butch" 41-29964
on a ferry flight bound for Belem Airfield (Belém) in
Brazil
and crashed southwest of Caux (Kaw), French Guiana (Guyana). The entire crew with the exception of Harmon were killed in the crash. Only
2nd Lt. Thomas D. Harmon
managed to bail out and was later rescued and returned to duty.
