Mission History On September 11, 1942 in the morning took off from Cairns Airfield piloted by 1Lt Robert G. Hazard as part of a ferry flight of P-40s from the 7th FS flying via Horn Island Airfield then onward to Port Moresby. Flying over Princess Charlotte Bay, this aircraft was last seen falling back from the formation and began loosing altitude and from roughly 100' made a steep dive into the sea and crashed. When this aircraft failed to arrive, it was officially listed as Missing In Action (MIA). Hazard was presumed lost due to an engine failure over Princess Charlotte off the eastern coast of the Cape York Peninsula and was declared Missing In Action (MIA). Search When observed to crash, the other planes in the ferry flight searched the area. Reportedly, the crashed aircraft was visible from the air but they saw no trace of the pilot. Afterwards, the rest of the formation proceeded to Coen Airfield to land and transmit a message to Horn Island, Port Moresby, Cairns and Townsville to report the loss. Memorials Hazard was officially declared dead on September 12, 1942. He did not earn the Purple Heart, posthumously as the flight he was lost was deemed non-combat. He is memorialized at Manila American Cemetery on the tablets of the missing. References Note, some sources incorrectly state Hazard was associated with the crash of a P-40E Warhawk on April 29, 1942 at Batchelor Field that resulted in the death of three personnel on the ground.
